Set at the Tojo University Hospital, the series follows Seishiro Tokai, a brilliant but arrogant surgeon known for his 100% success rate in complex procedures. Despite his surgical prowess, he is nicknamed 'The Surgeon Who Kills' by his colleagues due to his abrasive personality and his habit of mocking those who fail. He works in a department dominated by political maneuvering and the introduction of new, controversial surgical technologies. The plot centers on the conflict between Tokai and the hospital's ambitious administration, which seeks to replace traditional surgical skills with robotic systems. As Tokai navigates internal power struggles and medical ethics, he remains obsessed with a mysterious 'Black Pean'—a surgical instrument left inside a patient's body during a past operation—which holds the key to a dark secret involving his own family and the hospital's history.
